1. HOPE - Health Outcomes and Patient Experience
Patient Reported Measures 

Health Outcomes and Patient Experience (HOPE) is a purpose-built IT platform that enables patients to provide direct and timely feedback to their healthcare teams about outcomes and experiences that matter to them.

Portals:

Patient portal

Patients and/or carers can access their HOPE portals from any device anywhere using their Service NSW account to provide consent and complete the patient-reported measures surveys assigned to them.

Clinician Portal

The clinician portal enables clinicians to routinely collect and use patient-reported measures at the point of care.  They can then use this information for shared decision-making with their patient about their care and treatment plan.

eMR integration

HOPE has been integrated with the electronic Medical Record (eMR) across select LHDs and SHNs. This integration enables single sign on for clinicians and provides quick access to PRMs by making them viewable within the eMR alongside clinical data.

Patient portal

Patients are invited to provide feedback multiple times throughout the duration of their care. care teams now have data throughout different stages of the patient’s healthcare journey which is leading to more holistic care aligned with needs and preferences of patients.

All PRMs surveys have also been translated to support culturally safe and inclusive care for patients and also designed for use on different devices.

Clinician portal

Clinicians assign surveys and view the completed survey results in real time. They can then use this information for shared decision-making with their patient about their care and treatment plan.

These wireframe samples show the clinician view and the complexity of assigning multiple surveys within a clinical program.

EMR integration

Single sign on for clinicians provides quick access to PRMs by making them viewable within the eMR alongside clinical data. These wireframe samples show the complexity of the UX design: linking clinical data with patient surveys and appointments.

Challenges

2. School Vaccination Portal

Consent and Records Management for Immunisation (CaRMI) program

CaRMI launched in early 2023, we have been working with education authorities and schools to promote online consent for parents and carers. Promotional materials include translated resources for culturally and linguistically diverse communities.

Portals

Parent / carer portal

  • Ability to easily update the child's details and provide consent

  • Receive SMS notifications when vaccinations are given

  • Gain quicker access to the child's vaccination records from the Australian Immunisation Register (AIR).

Clinician Portal

  • Reductions in manual data entry

  • Improved ability to schedule clinics and allocate resources online

  • Improved functions to manage consent records

  • Ability to record vaccination details in real time, supporting a faster upload to the AIR

  • Access to automated reporting tools, replacing manual reporting processes.

Parent / carer portal

Healthcare stakeholders, parents and carers tested the solution during development.

Clinician portal

PHUs and CHCs use the portal to manage the vaccination program, schedule clinics and allocate resources. Vaccination clinic managers have expressed how much easier it is to set up clinics and manage resources. The portal has significantly reduced the amount of manual data entry for staff.

This wireframe sample is one of the many iterations to work out the challenge of visual elements, that are clinically safe and informative to the clinicians.
